머리 비듬의 개요

• 피부층은 표피층과 진피층으로 구성된다. 표피층의 맨 바깥층을 각질층이라고 한다.
• 각질은 각질층에서 정상적으로 계속 탈락되고 재생되는 것이 정상이다. 
• 어떤 원인으로 비정상적으로 각질의 더 많이 생성되고 더 많이 탈락 되고 각질이 평판 상으로 밀집되어있는 것을 비듬이라고 한다. 
• 이때 비듬이 피부층의 표피층 위에 붙어 축적될 수도 있고 쉽게 떨어질 수 있다. 
• 특별히 머리의 두피에 생긴 비듬을 머리 비듬이라고 한다.
• 비듬은 전신 피부 어디에서든지 생성될 수 있다. 

• • 지루성 피부염으로 생긴 비듬은 기름기가 있고 황색이다.
  • 건선으로 인해 생긴 비듬은 은색이고,
  • 어루러기(진풍)로 인해 생긴 비듬은 눈으로 보기가 힘들 정도로 작고
  • 어린선으로 인해 생긴 비듬은 눈으로 쉽게 볼 수 있을 정도로 크고 표피 각질층에 꼭 붙는 것이 보통이다.

• 일반적으로 원인에 따라 치료한다. 
• 머리 비듬 치료제는 비듬 치료 샴푸 등이 있다.
• 필요에 따라 매일 샴푸한다.
• 광물성 기름을 바르거나 부드러운 플라스틱 빗으로 빗는다.
• 필요에 따라 유황-살리실산제(Sulfur salicylic acid) 샴푸로 치료 한다. 
• 다음 각 항에서 비듬의 원인과 치료에 대해 구체적으로 설명한다. 

1. 지루성 피부염으로 인한 머리 비듬 Scalp Scaling or Dandruffs due to seborrheic dermatitis

• 지루성 피부염은 만성 염증성 피부염이다. 
• 생후 몇 주 내에서 몇 개월의 영아들에게 생기는 것이 보통이다.
• 영아기 이후에서 사춘기 이전까지 드물게 생긴다.
• 두피에 지루성 피부염이 있으면 두피에 홍반과 비듬이 생길 수 있다. 
• 그 비듬은 기름기가 있는 것이 보통이다.
• 두피, 목, 기저귀 차는 부위, 신체 간찰 부위의 피부에 주로 더 생긴다.
• 원인은 확실치 않다. 
• 경도 코르티코스테로이드제 연고나 로션 등으로 치료한다.

•  아토피성 피부염이 있거나 아토피성 체질인 사람의 두피, 손바닥, 발바닥 등의 피부에 작고 희고 마른 비듬이 더 잘 생길 수 있다. 

• 아토피성 피부염을 치료할 때와 거의 같이 아토피성 피부염 치료를 일반적으로 하고 필요에 따라 코르티코스테로이드제 크림이나 로션 등으로 치료한다.

3. 석면상 버짐으로 인한 머리 비듬 Scalp Scaling or dandruffs due to Tinea amiantacea

• 두피에 생긴 비전염성 국소적 피부염의 일종이다. 
• 이 병으로 생긴 비듬은 은백색 또는 회색이고 비듬이 조밀하게 모간에 붙어 있는 것이 보통이다. 
• 치료하기가 어려우나 코르티코스테로이드제 연고나 로션으로 치료한다. 

4. 조직구 중식증으로 인한 머리 비듬 Scalp Scaling or dandruffs due to histiocytosis

• 혈액 조직구 이상으로 생기는 병이다. 
• 이 병이 있으면 농포, 소 결절, 습진성 구진, 소양감 등의 증상이 두피에 나타날 수 있다. 
• 지루성 피부염이 있을 때 생기는 비듬과 비슷한 비듬이 두피에 많이 생길 수 있다. 
• 조직구 증식증을 치료하면 비듬도 자연히 없어진다.

Overview of Hair Dandruff

• The skin layer consists of the epidermis and dermis. The outermost layer of the epidermis is called the stratum corneum.

• It is normal for keratin to continue to fall off and regenerate normally from the stratum corneum.

• For some reason, more and more keratin is abnormally produced and more keratin is removed, and dandruff is called dandruff.

• At this time, dandruff can adhere to and accumulate on the epidermis layer of the skin layer and can easily fall off.

• Dandruff, especially on the scalp, is called hair dandruff.

• Dandruff can form anywhere on the skin of the body.

• o Dandruff caused by seborrheic dermatitis is oily and yellow.

o Dandruff caused by psoriasis is silvery,

o Dandruff caused by dandruff is so small that it is difficult to see with the naked eye.

o Dandruff caused by ichthyosis is large enough to be easily seen with the naked eye and is usually attached to the stratum corneum of the epidermis.

• Treatment is usually based on the cause.

• Hair dandruff treatment includes dandruff treatment shampoo. • Shampoo daily as needed.

• Apply mineral oil or comb with a soft plastic comb. • Treat with Sulfur salicylic acid shampoo as needed.

1. Scalp Scaling or Dandruffs due to seborrheic dermatitis

• Seborrheic dermatitis is a chronic inflammatory dermatitis.

• It is common in infants between a few weeks and a few months of age. • Rarely occurs after infancy and before puberty.

• Seborrheic dermatitis on the scalp can cause erythema and dandruff on the scalp.

• The dandruff is usually greasy.

• It is more common on the skin of the scalp, neck, diaper areas, and intersecting areas of the body. • The cause is unclear.

• Treat with mild corticosteroid ointment or lotion.

2. Scalp Scaling or dandruff due to atopic dermatitis

• People with atopic dermatitis or atopic constitution are more likely to develop small, white, dry dandruff on the skin of the scalp, palms and soles of the feet.

• Treat atopic dermatitis in the same way as when treating atopic dermatitis in general and, if necessary, treat with corticosteroid cream or lotion.

3. Scalp Scaling or dandruffs due to Tinea amiantacea

• It is a type of non-infectious topical dermatitis on the scalp.

• Dandruff caused by this disease is usually silvery white or gray and densely attached to the hair shaft.

• It is difficult to treat, but it is treated with corticosteroid ointment or lotion.

4. Scalp Scaling or dandruffs due to histiocytosis

• It is a disease caused by abnormalities in blood histiocytosis.

• With this disease, symptoms such as pustules, small nodules, eczematous papules, and itching may appear on the scalp.

• Dandruff similar to the dandruff that occurs when you have seborrheic dermatitis can occur on your scalp a lot.

• When histiocytosis is treated, dandruff will disappear naturally.
